Early Help Universal Plus Referral form

Where there is no need for a full assessment, referrals can be made directly to a Child and Family Early Help Hub, via this form.

Referrals are sent to the relevant Early Help Hub. They offer a range of brief interventions, where 6 to 12 sessions of support can be provided. This may include access to:

  • evidence-based parenting programmes
  • domestic abuse recovery groups (for children or mothers)
  • young person / parent mediation to improve family relationships
  • low- level parental conflict work
  • targeted group or 1 to 1 work with young people around keeping safe, healthy relationships, self-esteem, as well as supporting families navigate services/signposting

This form is also used to access BICs and MOPAC mentoring.

Please note: Family consent is required to complete referral.

If a child needs support from more than two agencies, or from a service providing ‘targeted’ intervention, please use our Safeguarding or early help request form and select "Early help request".

If you have any immediate safeguarding concerns, please complete our Safeguarding or early help request.

Make sure you have the child and families details to hand before you start.
